I understand and agree that:

the information provided in this form is true and accurate to the best of my knowledge;

the submitted work is my own original work, or I have obtained the necessary permission to submit and present it;

submission is free, but a participation fee will apply if my work is selected;

selection is subject to the decision of the festival’s curatorial team, and submission does not guarantee participation;

if selected, I agree to provide any additional files, information or materials required by the organiser within the stated deadline;

if I am an international visual-art participant, I understand that my selected artwork will be reproduced locally in Malaysia and that the applicable participation fee is USD85 per selected artwork;

I understand that the standard international reproduction sizes are 16" × 16" (40.64 × 40.64 cm), 30" × 20" (76.2 × 50.8 cm), or 20” X 30” (50.8 X 76.2 cm) with the final size determined by the curatorial team;

I understand that diagnosis and participant-profile information may be collected for selection, programme planning and impact evaluation, and will be treated as confidential unless I give permission for public disclosure;

if AI or other significant assistance was used in creating my work, I have declared this accurately;

if my work is selected, I grant the organiser permission to display, photograph, reproduce and use images or excerpts of the work for festival-related exhibition, publicity, social media, website, programme materials, documentation and reporting, while copyright remains with the creator;

if I choose to make my work available for sale, I understand that the festival will retain a 20% commission on completed sales;

I understand that the organiser may make reasonable changes to the programme, exhibition layout, display format, venue or schedule where necessary; and

if the participant is under 18 years of age, I confirm that I am the parent or legal guardian, or that the required parent/guardian consent has been obtained.

提交此报名表，即表示我确认已经阅读、理解并同意 The Human Spectrum Music & Art Festival 2027: Beyond The Label 的相关条款与细则。

我理解并同意以下事项：

我在此报名表中提供的资料，在我所知范围内均属真实及准确。

我所提交的作品为本人原创作品，或我已取得所需的授权及许可，可以提交及展示该作品。

投稿本身不收取费用，但若作品成功入选，则需缴付相应的参与费用。

所有作品均须经过艺术节策展团队审核与遴选，投稿并不代表一定能够入选。

如作品成功入选，我同意在主办单位指定的期限内，提供所需的补充资料、文件、影像、音频或其他相关材料。

如果我是国际视觉艺术参与者，我明白入选作品将由主办单位在马来西亚进行本地复制制作，并需缴付 每件入选作品 USD85 的参与费用。

我明白国际视觉艺术作品的标准复制尺寸为 16" × 16"（40.64 × 40.64 cm） 或 30" × 20"（76.2 × 50.8 cm），最终尺寸将由策展团队根据作品及展览需要决定。

我明白主办单位可能会收集与参与者类别及诊断相关的资料，用于作品遴选、节目规划及影响评估。这些资料将被保密处理，除非我另行同意，否则不会公开披露。

如我的作品使用了 AI，或在创作过程中获得了重要协助，我确认已在报名表中如实申报。

如果作品成功入选，我同意主办单位可为艺术节相关用途展示、拍摄、复制及使用作品的图片、片段或相关内容，包括展览、宣传、社交媒体、官方网站、节目册、活动记录及报告。作品版权仍归创作者本人所有。

如果我选择让作品在艺术节期间出售，我明白主办单位将从已完成的销售中收取 20% 销售佣金。

我明白主办单位在有需要的情况下，可能会对活动流程、展览布置、展示方式、场地或时间安排作出合理调整。

如果参与者未满18岁，我确认已取得家长或法定监护人的同意；如本人为家长或法定监护人，我确认同意该参与者参加本次活动及提交作品。
